This addiction treatment program gradually transitions clients from residential care to a partial hospitalization program (PHP) and an evening intensive outpatient program (IOP) as they gain independence. Housing and transportation are provided for clients in the residential program or PHP. During the IOP, they return home or transition to nearby sober living, with the daytime available for work or job searching while attending evening programming. Programming addresses underlying co-occurring mental health concerns and trauma, including medications for addiction treatment (MAT) and access to medical and psychiatric providers. What to expect

Developing your care plan

  • Initial assessment: Care begins with an in-depth medical, psychological, and social assessment that guides the development of the care plan.
  • Adjustments as needed: Care plans can be adjusted if relapse occurs during treatment.
  • Various approaches to care: Treatment explores why a client turns to substances and addresses underlying mental health concerns using a combination of approaches. Approaches to treatment may include MAT, individual and group therapy, and family therapy alongside 12-Step meetings.

Who you will meet with

  • Primary therapist: The primary therapist works with clients during one-on-one therapy sessions.
  • Medical providers: Medical providers are on-site to provide care as needed.
  • Psychiatric providers: Psychiatric providers offer mental health evaluations and support for mental health challenges.
  • Volunteers: Volunteers from outside organizations come into the program to lead Alcoholics Anonymous (AA), Cocaine Anonymous (CA), and Heroin Anonymous (HA)-style meetings in the evenings.

Typical schedule

  • Morning and afternoon: Clients wake up, have breakfast, and the first medication pass is around 8 a.m. Therapy groups run from 9 a.m.-3 p.m. with breaks. Individual therapy sessions and medical and psychiatric appointments also occur during this time.
  • Free time: Clients have free time after 3 p.m. when they can swim, exercise, play games, relax, or watch TV.
  • Evening: Around 6:30-7 p.m. visiting volunteers hold AA, CA, or HA meetings that last about an hour. This is followed by another medication pass followed by personal time and bedtime.

Living experiences and amenities

  • House-based living: Clients stay in one of two co-ed residential houses that follow the same general approach to programming. There are 21 beds combined, and each house has a backyard pool.
  • Recreation: Recreation includes free weights, stationary bikes, ping-pong, and mini-golf.
  • Full kitchens: Clients have access to full kitchens where they can cook individually or prepare meals together.
  • Family contact: In-person visitation is not permitted during residential treatment. Clients also have a 1–2-week phone blackout period. After that, clients can make calls between 4-6 p.m. daily. This changes when clients transition into the PHP, when they receive an hour of cell phone access during lunch and four-hour off-site family visits on the weekends.

Aftercare

  • Partial hospitalization program: Clients spend 30–45 days in residential treatment and then transition into the PHP for another 30–45 days. During PHP, they continue living in Nirvana’s residential housing and are transported to the treatment center for daytime programming from approximately 9 a.m.-3 p.m.
  • Intensive outpatient program: After PHP, clients transition into the IOP. They return to home or transition to nearby sober living, with Nirvana providing placement assistance when needed. In the IOP, programming shifts to 5:30-8:30 p.m, Monday to Friday.
  • Practical and family support: Additional support can also include job readiness, budgeting, and time-management skills. Family workshops and counseling can also help clients rebuild relationships and strengthen their support system.
