Short-Term Residential Care for Men With Co-Occurring Needs

Horizons is a short-term, intensive residential substance use program for adult men, including those with co-occurring mental health concerns. Operated by non-profit Recovery Network of Programs, it provides 24-hour structure and supervision for adults, including those with complex medical or psychiatric histories. The program gives clients a focused period of stability, daily routines, and therapeutic support. Services are also available in Spanish.

Evidence-Based Therapies and MAT

Clinical services at Horizons rely on evidence-based approaches such as cognitive behavioral therapy (CBT), motivational interviewing, the Matrix Model, trauma-focused counseling, anger management, relapse prevention, and 12-step work. Men participate in individual, group, and family or couples counseling to address substance use, mental health symptoms, and related stressors. The program also offers medications for addiction treatment (MAT), as well as medication support for mental health conditions and nicotine dependence.

Whole-Person Support and Practical Services

Care begins with comprehensive assessments of substance use, mental health, physical health history, and tobacco use, which help the team match services to each person’s needs. Horizons integrates mental health services, case management, social skills development, and suicide prevention support so clients are not navigating these challenges alone. The program also addresses other addictions such as gambling and related behaviors. Transportation assistance can reduce practical barriers to starting and staying in treatment.

Preparing for Life After Treatment

A core goal of Horizons is helping clients build stability after they leave residential care. Staff work with each man on discharge planning and aftercare, connecting him with outpatient treatment, community mental health supports, housing services, and self-help groups. Recovery coaching, peer mentoring, and alumni involvement give clients a chance to build a lasting support network. The program also offers life skills development and job-related support. Naloxone and overdose education, along with outcome follow-up, reinforce safety and ongoing engagement in recovery once clients return to their communities.
