Step-Down Support Through Outpatient Care and Statewide Telehealth

Telehealth services are delivered through an app-based platform and may include individual, family, and group therapy; treatment planning and reviews; coordination with primary care; recovery support; and medications for addiction treatment (MAT).

Treatment includes individual, group, and family counseling, structured groups, and educational offerings. The program also includes wellness-focused services such as acupuncture, along with trauma-related counseling and tobacco screening and cessation counseling.

Services include case management, peer support and recovery coaching, housing support, help accessing social services, employment counseling or training, and transportation assistance.
