Family-Centered Care for Mothers and Children

The HOME Program provides residential treatment designed specifically for expecting, pregnant, and parenting women. One of the few programs of its kind in Southern Oregon, it allows mothers to live with their children, ages five and under, throughout recovery. Operated by OnTrack Rogue Valley, a long-standing nonprofit serving the region since 1969, the program emphasizes family connection as a cornerstone of healing.

Supportive Home-Like Environment

Located in Medford, the program offers a comfortable, family-oriented setting that balances structure with warmth. Mothers share living spaces with their children, with access to a licensed childcare center, outdoor play areas, a kitchen and dining space, and walking paths surrounded by greenery. These amenities create a safe, familiar environment that supports both recovery and daily family life.

Integrated, Evidence-Based Treatment

Treatment incorporates cognitive behavioral therapy (CBT), motivational interviewing, relapse prevention, and trauma counseling, addressing both substance use and co-occurring mental health concerns such as depression, anxiety, and post-traumatic stress. Clients receive individual, group, and family counseling, along with health education and medications for addiction treatment (MAT) coordinated through licensed prescribers. The trauma-informed approach helps mothers regain emotional stability and practical coping skills.

Building Long-Term Stability

From admission through discharge, each mother works closely with a care team to plan next steps for recovery and independence. The program provides housing and social services support, recovery coaching, transportation assistance, and aftercare coordination to help families transition successfully. Education on overdose prevention, tobacco cessation, and wellness is built into care planning, empowering mothers to sustain recovery, maintain custody, and create safe, stable homes for their children.
