These elements are intended to help clients build routines, practice coping skills, and strengthen day-to-day habits that can support recovery after discharge.

Community Connection and Transition Planning

Clients are encouraged to explore community support options, including 12-step and Celebrate or Life Recovery meetings, and to explore their own spirituality in a way that fits them. Transitional and recovery supports include discharge planning, aftercare/continuing care, outcome follow-up after discharge, naloxone and overdose education, peer support or mentoring, recovery coaching, housing-related services, and help obtaining social services. Transportation assistance is also offered.

Medication Support When Indicated

When appropriate, the program can help clients access medications for addiction treatment (MAT) that support recovery from opioid and alcohol use disorders, alongside counseling and other treatment approaches. Nicotine replacement and tobacco-use screening and counseling are also available.
