City-Based Continuum of Care

This nonprofit campus in Lake City provides step-up and step-down options for substance use and co-occurring mental health needs, including hospital inpatient stabilization, residential treatment, intensive outpatient (IOP), regular outpatient, and telehealth. Care coordination helps people move between levels as needs change while staying connected to the same organization.

Adolescent Residential Program With School Support

The Recovery Center is a six-month program for ages 13–17. Treatment combines 12-step facilitation, motivational enhancement, and cognitive behavioral therapy (CBT). Students attend school through the local district, with GED and credit-recovery options. The program emphasizes coping and problem-solving skills, with a short-term Respite Center that supports caregivers.

Certified Opioid Treatment and Evidence-Based Therapies

A federally certified opioid treatment program (OTP) pairs counseling with medications for addiction treatment (MAT) such as methadone, buprenorphine, with or without naloxone, and extended-release naltrexone. Across programs, clinicians offer CBT, motivational interviewing, relapse prevention, anger management, and trauma-focused counseling in person and via telehealth. Health education, screenings, metabolic monitoring, hepatitis B vaccination, and nicotine-cessation supports are available within a tobacco-free policy.

Access, Practical Supports, and Policies

Meridian accepts insurance and self-pay with sliding-fee options, and offers case management for benefits, housing referrals, peer mentoring, and optional spiritual support. Services include naloxone and overdose education, discharge planning, aftercare, and follow-up. Tobacco and vaping are not allowed, and some programs permit cell phones.
