Overview
WellFully Premier Adolescent Care Center provides trauma-informed, community based mental health and substance use care for adolescents in Rapid City, South Dakota. Both residential and outpatient programs are offered. Treatment includes individual counseling adapted to each teen’s needs, family counseling, and peer support. A walk-in crisis center is available for teens in distress who need immediate intervention. Payment assistance and a sliding fee scale are provided to help families access care.

Integrated Substance Use Recovery and Mental Health Treatment
Clients can access several levels of care under one umbrella. The Adolescent Addiction Recovery Unit offers inpatient and outpatient substance use treatment, with 24-hour supervision in residential care and intensive or regular outpatient services. Clinical approaches can include cognitive behavioral therapy (CBT), motivational interviewing, Matrix Model groups, anger management, relapse prevention, trauma-related counseling, and 12-step work. The center offers medication for mental health conditions and nicotine replacement, and it can work with clients who receive medications for addiction treatment (MAT) from outside prescribers.
24/7 Adolescent Crisis Support
WellFully’s Adolescent Crisis Care Center is a 24/7 walk-in program for youth in emotional crisis, including those who may be considering suicide. It is dedicated specifically to middle and high school-aged youth in crisis. The center offers immediate, short-term support in a supervised, therapeutic setting, with suicide prevention services and connections to ongoing care. No referral or appointment is required, which can make it easier for families to get help in urgent situations.
Trauma-Informed, Culturally Responsive Care
The treatment philosophy is intentionally trauma-informed. Services are built around safety, trust, and empowerment, which can be especially important for youth who have experienced poverty, abuse, neglect, or other traumatic events. Programming includes specialized groups for grief, anger, intimate partner and domestic violence, and other forms of trauma. Care is culturally responsive so that youth feel seen and respected in their identities, and the center works collaboratively with families, schools, healthcare providers, and community partners to coordinate support.
Education, Life Skills, and Aftercare
Education and future planning are central to WellFully’s model. On-site academic support, an after-school and summer program, and access to GED preparation or college scholarships help clients stay connected to school and long-term goals. Vocational training, employment counseling, and job-readiness services support economic stability after treatment. Additional recovery supports include self-help groups, mentoring, peer support and outcome follow-up after discharge. Families can access case management, which can assist with housing and social services. Transportation assistance can be arranged.
