Structured Men’s Residential Care
Transition House provides residential substance use treatment for adult men in St. Cloud, Florida. Clients live onsite in a full-service residential environment where treatment and housing are integrated, and meals are provided as part of the program.
Integrated Support for Substance Use and Mental Health
The facility provides treatment for substance use along with care for co-occurring mental health needs. Clients can receive support for concerns such as depression, anxiety, and post-traumatic stress, with access to professional psychiatric evaluation services. Counseling and education are combined with evidence-based approaches, including cognitive behavioral therapy (CBT), motivational interviewing, trauma-related counseling, relapse prevention, anger management, and 12-step facilitation. This blend is designed to help clients understand how mental health symptoms and substance use can reinforce each other, and to build practical strategies for managing cravings, stress, and trauma responses in daily life.
High-Intensity Group Therapy and Individual Counseling
Group therapy is a core part of the program, with an intensive weekly schedule that includes about 20 hours of group therapy per week. Clients also receive individual counseling and are paired with a counselor who supports progress throughout the stay. In addition to group and one-on-one sessions, the facility provides substance use disorder education, recovery planning, and skills-focused support aimed at helping men practice coping tools in real time. Family counseling and couples counseling are also offered.
Medication Support and Safety Planning
Transition House provides medications for addiction treatment (MAT) and also offers medication services for mental health conditions. The facility can accept clients who are already using MAT prescribed elsewhere, and some prescribing may be coordinated through a contracted prescribing entity. As part of planning for safer transitions, the facility provides overdose education and naloxone-related support, alongside discharge planning and continuing care planning.
Practical Reentry Support, Veterans Services, and Continuing Care
Beyond therapy, the program emphasizes stability and self-sufficiency after residential treatment. The facility provides case management, aftercare/continuing care planning, outcome follow-up after discharge, and recovery supports such as mentoring or peer support. Clients may also receive help with housing services, assistance obtaining social services, and employment counseling or training. Transition House also offers specialized programming for veterans and members of military families, including services tied to housing stability and case management.
