A Bridge from Incarceration to Re-entry

The program is offered at no cost so men can focus on stabilization and next steps. On-site services include substance use counseling delivered by Healthcare Alternative Systems, mental health support through Adler, and physical healthcare through RUSH. Clients have shared or single rooms with meals, a library, recreation rooms, a weight room, and free laundry. The program welcomes men who need a structured bridge to community.

Phased Approach to Independence

The design follows a phased model that typically lasts six to nine months and adapts to individual needs. An integrated case management approach coordinates care across disciplines and keeps goals realistic, measurable, and focused on independence. The sequence moves from orientation to core programming, then to employment and housing readiness, followed by transition and aftercare. Case managers help clients set plans for health, recovery, education, employment, and housing.

Establishing Structure and Fulfilling Wraparound Needs

The early orientation phase covers assessments, securing identification, and applying for benefits such as SNAP and Medicaid. Next, core programming emphasizes substance use recovery, healthcare needs, and psychological or psychiatric assessment as needed. Financial and computer literacy are introduced early to support long-term job and housing access. Clients also complete housing needs assessments to prepare for the next phase. The predictable routine of meals, room assignments, and onsite amenities supports participation and stability.

Empowering Men to Succeed After Saint Leonard’s

Employment and housing readiness includes job search and training, resume and interview preparation, and coordination with education or vocational programs. Clients learn tenant rights and responsibilities, as well as how to budget for rent, utilities, and other essentials. Case managers assist with applications for transitional or permanent housing and coach men on being reliable neighbors and leaseholders. Mentoring, community resource connections, and alumni involvement help sustain progress after discharge. The program’s focus on daily habits—work, health, recovery, and finances—translates treatment gains into real-world stability. Aftercare encourages men to stay connected through the Saint Leonard’s alumni network.
