Located in Carbondale in Colorado’s Rocky Mountain setting, Jaywalker’s Outpatient Program is designed for adult men continuing substance use treatment while living more independently. The program reflects Jaywalker’s open community model and 12-step foundation, emphasizing integrity, personal accountability, and service as part of ongoing recovery.

Evening Groups and Weekly Individual Therapy Focused on Relapse Prevention

This fully licensed 90-day outpatient program includes two evening group therapy sessions each week and weekly individual therapy. Counseling centers on relapse prevention and recovery promotion, combining education, therapy, and group-based work to help men apply skills in real-life situations while maintaining progress made in earlier treatment.

Integrated Support for Co-Occurring Needs and Medications for Addiction Treatment

Jaywalker provides substance use treatment for adult men, including support for co-occurring mental health and trauma-related needs. Services may include approaches such as cognitive behavioral therapy (CBT), motivational interviewing, trauma-related counseling, and 12-step facilitation, with telehealth therapy also available. The program can incorporate medications for addiction treatment (MAT), including buprenorphine and naltrexone, and may provide medications for alcohol use disorder.

Family Case Management, Alumni Connection, and Practical Step-Down Supports

Clients receive ongoing family case management designed to fit their needs, along with recovery check-ins to support continued progress. The program also strengthens connection to Jaywalker’s alumni network and the local recovery community for longer-term accountability. Transportation assistance can help reduce barriers to attendance, and the outpatient track is coupled with Jaywalker’s sober living option reserved for men graduating from the Jaywalker Solutions Program.
