What to expect

Developing your care plan

• First steps: Care planning begins with an evaluation that considers many factors, such as general health, withdrawal potential, and readiness for change.
• Ongoing assessments: These factors are evaluated and tracked over time and used to assess the effectiveness of resulting care plans and make changes as needed.
• Adjustments and outside resources: Clinicians adjust treatment plans based on client progress, taking strengths and challenges into consideration. Clients can also get support from case managers who connect clients with additional resources when additional support is needed.

Who you will meet with

• Counselors: Clients meet with master’s-level counselors for individual and group therapy sessions.
• Case managers: Case managers help connect clients with community resources to meet practical needs.
• Medical providers: Medical providers manage MAT services, monitor progress, and make changes to treatment as needed. Typical schedule
• Appointment-based care: Clients come to the facility during the week; the frequency of visits is determined by each individual’s care plan.
• Adult services: Adults in the program may attend individual substance use counseling sessions, group counseling, or programming focused on harm-reduction strategies. Case management services and peer support are also available.
• Youth program: The youth program is a 6- to 12-week outpatient program for people ages 10–18. Clients attend individual counseling focused on healthier lifestyles and harm reduction. Families can also participate in family counseling to rebuild communication and improve relationships.
