Developing your care plan

• Personalized treatment planning: During admission, clients undergo an in-depth clinical assessment that is used to guide a personalized care plan. Treatment plans may include individual, group, and family therapy, psychiatric care, medication management, trauma-focused therapies, and relapse prevention. Clear recovery goals help treatment stay on track and provide clinical accountability.
• Adjusting care as needs change: The clinical team keeps track of each client's progress and adjusts the care plan as needs change. New goals can be incorporated as progress is made or additional concerns become apparent.

Who you will meet with

• A multidisciplinary treatment team: Clients may work with counselors, therapists, clinical supervisors, social workers, educators, and physicians. Psychiatric and addiction medicine professionals may also be available to provide additional support.
• Communication across the care team: Staff members work together and communicate frequently to coordinate addiction treatment, medication needs, mental health care, and trauma treatment. They have experience and training in a variety of therapeutic approaches and skills-building practices that can help clients during treatment.

Typical schedule

• Flexible levels of outpatient care: The PHP typically takes place Monday through Friday for five to six hours a day, or about 25 hours a week. The IOP provides more than nine but fewer than 25 hours a week, and standard outpatient care takes place fewer than nine hours weekly. Each level allows clients to receive treatment while staying at home or in supportive housing. The weekly time commitment decreases as care becomes less intensive.
• A structured treatment day: Much of the time in PHP is spent in group therapy, with individual counseling taking place throughout the week. Clients may also take part in family sessions and educational programs. Medical appointments may be included, as well as medication management when determined to be appropriate.
• Integrated addiction and mental health care: Programming supports clients with substance use concerns as well as co-occurring mental health conditions, trauma, and family dysfunction. Care may include evidence-based therapies such as cognitive behavioral therapy (CBT), dialectical behavior therapy (DBT)-informed care, and narrative therapy. Psychiatric care and medication management are also available when determined to be appropriate.
• Trauma and medication support: Trauma-informed therapists support clients during individual sessions. Eye movement desensitization and reprocessing (EMDR) and other trauma-focused approaches may help clients understand experiences that are connected to their substance use and mental health symptoms. Medications for addiction treatment (MAT), such as buprenorphine, naloxone, and naltrexone, may also be used. Clients receiving medication support also take part in counseling to support their treatment.

Aftercare

• Additional support and community: In addition to being able to transition through PHP, IOP, and standard outpatient care as their needs change, clients have continuing access to individual, group, and family therapy, holistic services, and connections with the alumni community.
