Developing your care plan

Clients are then placed into PHP, IOP, standard outpatient, virtual care, or evening programming. - Collaborative treatment planning: Clients can expect individualized care plan development with an ongoing weekly review during one-on-one sessions with a licensed therapist. This includes monitoring of recovery goals, mental health symptoms, relapse risks, and family or social functioning.
Who you will meet with

Typical schedule
- Partial hospitalization programming (PHP): A full day of programming from approximately 9 a.m. to 3 p.m. is available for PHP clients. The treatment schedule throughout the day can include group therapy, psychoeducation, relapse prevention, mental health support, recovery planning, individual therapy, and medication management.
- Intensive outpatient programming (IOP): The IOP programming has an evening option from 6 p.m. to 9 p.m. designed for clients balancing work, school, or family responsibilities. It also includes structured group therapy sessions, a weekly individual therapy session, and medication management as needed.
- Virtual care and ongoing support: Virtual treatment options, including standard and IOP treatment, are available to provide flexible participation options alongside daily responsibilities. Individual therapy and group support are available virtually.
Aftercare
- Aftercare planning and recovery support: Aftercare planning provides support in recovery coaching, relapse prevention planning, and ongoing accountability. Clients can also receive connection to peer support and continued access to recovery resources as they transition toward increased independence.
